Job Summary




  • We are seeking a dedicated and compassionate Community Health Extension Worker (CHEW) to support the provision of quality primary healthcare services and community-based health interventions. The successful candidate will play a vital role in promoting health education, disease prevention, patient advocacy, and access to healthcare services within the community and healthcare facility.

  • This position requires a proactive individual with strong interpersonal skills and a passion for improving health outcomes through preventive and community-focused healthcare initiatives.



Key Responsibilities




  • Deliver basic healthcare services in accordance with approved clinical guidelines.

  • Conduct patient assessments and provide appropriate health education.

  • Support immunization, maternal and child health, family planning, and preventive healthcare programs.

  • Participate in health promotion campaigns and community outreach activities.

  • Conduct health screenings and assist in identifying individuals requiring further medical attention.

  • Provide counseling on health-related issues, including hygiene, nutrition, and disease prevention.

  • Maintain accurate patient records, registers, and activity reports.

  • Monitor and follow up on patients requiring ongoing community-based care.

  • Support referral processes and coordinate follow-up care where necessary.

  • Participate in disease surveillance and reporting activities.

  • Collaborate with healthcare professionals to ensure continuity of patient care.

  • Ensure compliance with infection prevention and control measures.

  • Support quality improvement initiatives within the healthcare facility.

  • Participate in staff training, meetings, and continuous professional development activities.



Requirements




  • CHEW qualification from a recognized institution.

  • Valid practicing license.

  • Minimum of 2–3 years of relevant clinical experience.

  • Sound knowledge of primary healthcare principles and community health practices.

  • Experience in health education, outreach, and preventive healthcare programs.

  • Strong communication, interpersonal, and counseling skills.

  • Good documentation and reporting abilities.

  • Ability to work independently and as part of a multidisciplinary team.

  • Strong commitment to patient-centered care and community development.
