Job Objective




  • To provide pharmaceutical care services to patients within the hospital, ensure safe, effective, and evidence-based medication therapy management to optimize patient outcomes.

  • The job holder shall be responsible for drug distribution, and provision of clinical services, and may be required to perform functions not expressly stated.



Responsibilities




  • Always maintain the drug stock in the hospital. This includes storing medications safely, and monitoring availability (low in stock) to prevent out-of-stock.

  • Check prescriptions for appropriateness and proper dosage and communicate with prescriber in situations where dosage adjustment is required, or an alternative is suggested.

  • Provide advice on the dosage of medicines and the most appropriate form of medication to healthcare professionals.

  • Dispense prescribed medication to patients, advise them on the indications, contraindications, druginteractions or food-drug interactions, side effects, and dosage, and state the need for compliance to the dosing regimen when necessary.

  • Compound of extemporaneous preparations.

  • Maintain a database of patient drug history, and proper inventory control of medicines available in the pharmacy.

  • Supervise the work of less experienced and less qualified staff on duty.

  • Attend to all drug information requests from other members of the healthcare team, patients, and the public.

  • Keep up to date with and contribute to research and development.

Required Skills / Competencies and Qualification




  • Strong clinical knowledge and expertise in pharmacotherapy, medication management, and pharmaceutical care principles.

  • Proficiency in using pharmacy information systems, electronic medical records (EMR), and clinical decision support tools.

  • Excellent communication, interpersonal, and teamwork skills with the ability to collaborate effectively with diverse healthcare professionals and patients.

  • Commitment to continuous professional development, evidence-based practice, and quality improvement in pharmacy services.

  • Demonstrated leadership abilities, problem-solving skills, and attention to detail in managing complex medication-related issues and patient care scenarios.

  • Detail-oriented with a focus on patient safety.

  • Minimum of 2-3 years of pharmacy practice experience in a well-recognized establishment.

  • Must have a valid practice license from the Pharmacy Council of Nigeria (PCN).

  • Must possess a minimum of Bachelor's (B.Pharm) or D/Pharm in Pharmacy from a well-recognized and reputable institution.
