Job Summary
The Home Care Nurse provides high-quality nursing care to clients in their homes, ensuring their medical needs are met while promoting independence and well-being.
The role involves traveling to clients’ homes, assessing their health conditions, administering treatments, providing education and collaborating with healthcare professionals and family members to ensure comprehensive care.
Key Responsibilities
- Administer medications, treatments, and other medical interventions, following physician orders and nursing protocols.
- Conduct regular assessments of clients' health status.
- Perform wound care, catheter care, injections, IV therapy, and other clinical procedures as required.
- Monitor changes in clients’ health and report any significant changes to the supervising nurse.
- Maintain accurate, up to date clinical documentation, including assessments, treatment notes, and medication administration records.
- Collaborate with other healthcare providers, such as doctors, physical therapists, and social workers, to develop and adjust care plans.
- Respond promptly to emergencies and provide appropriate medical interventions, including first aid or emergency procedures.
Qualifications & Experience
- Bachelor’s Degree in Nursing (B.Sc. Nursing) and equivalent nursing qualifications.
- Valid nursing license and registration with the relevant nursing council.
- Minimum of 5 years of nursing experience, ideally in home care, geriatrics, community nursing or related fields.
- Valid driver’s license is required.
- Experience in wound care, medication management, and home-based medical care is an advantage.
- CPR and First Aid certifications are desirable.
