Role Summary:
As a Clinical Pharmacist, you will be responsible for providing pharmaceutical care to patients, optimizing medication therapy, and ensuring safe and effective medication use.
Responsibilities:
- Review patients' medication regimens and medical histories to identify drug-related problems and optimize therapy.
- Collaborate with healthcare providers to develop and implement individualized treatment plans.
- Provide medication counseling and education to patients on proper medication use, side effects, and interactions.
- Monitor patients' responses to medication therapy and adjust treatment plans as needed.
- Participate in medication reconciliation and medication therapy management programs.
- Ensure compliance with regulatory standards and pharmacy policies and procedures.
Requirements:
- Bachelor's or Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Valid license to practice as a Pharmacist in Nigeria.
- Proven experience working as a Clinical Pharmacist in a hospital or clinical setting.
- Strong knowledge of pharmacotherapy principles and practices.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a multidisciplinary healthcare team.
