Role Summary:

As a Clinical Pharmacist, you will be responsible for providing pharmaceutical care to patients, optimizing medication therapy, and ensuring safe and effective medication use.

Responsibilities:

• Review patients' medication regimens and medical histories to identify drug-related problems and optimize therapy.


• Collaborate with healthcare providers to develop and implement individualized treatment plans.


• Provide medication counseling and education to patients on proper medication use, side effects, and interactions.


• Monitor patients' responses to medication therapy and adjust treatment plans as needed.


• Participate in medication reconciliation and medication therapy management programs.


• Ensure compliance with regulatory standards and pharmacy policies and procedures.

Requirements:

• Bachelor's or Doctor of Pharmacy (PharmD) degree from an accredited institution.


• Valid license to practice as a Pharmacist in Nigeria.


• Proven experience working as a Clinical Pharmacist in a hospital or clinical setting.


• Strong knowledge of pharmacotherapy principles and practices.


• Excellent communication and interpersonal skills.


• Ability to work collaboratively in a multidisciplinary healthcare team.
