Role Summary:


As a Patient Care Coordinator at our client's healthcare facility, you will be responsible for coordinating and managing patient care activities to ensure the delivery of high-quality and efficient healthcare services. You will serve as a liaison between patients, healthcare providers, and support staff to facilitate continuity of care and positive patient experiences.


Responsibilities:



  • Coordinate patient appointments, admissions, and discharges, ensuring timely access to healthcare services and continuity of care.

  • Collaborate with physicians, nurses, and other healthcare providers to develop and implement individualized care plans for patients.

  • Communicate with patients and their families to provide information about treatment plans, procedures, and healthcare resources.

  • Coordinate referrals to specialty services, diagnostic tests, and ancillary services as needed to meet patient needs.

  • Monitor and follow up on patient progress and outcomes, addressing any concerns or issues that may arise during the course of care.

  • Maintain accurate and up-to-date patient records and documentation in the electronic health record system.

  • Provide support and assistance to patients and families in navigating the healthcare system and accessing community resources.


Requirements:



  • Bachelor's degree in Healthcare Administration, Nursing, or related field.

  • Minimum of 2 years of experience in healthcare coordination or patient advocacy role.

  • Strong interpersonal and communication skills, with the ability to interact effectively with patients, families, and healthcare providers.

  • Knowledge of healthcare regulations, patient rights, and ethical standards.

  • Ability to prioritize tasks, problem-solve, and work independently in a fast-paced environment.

  • Proficiency in Microsoft Office and electronic health record (EHR) systems.
