As a Case Manager (Rehabilitation) Volunteer, you will play a vital role in supporting individuals through their rehabilitation process.

Key Responsibilities:
Conduct initial assessments to determine clients' needs, goals, and strengths.
Develop individualized rehabilitation plans in collaboration with clients, incorporating input from relevant stakeholders.
Coordinate access to necessary resources, including medical care, therapeutic services, housing, and community support programs.
Advocate for clients' rights and needs within various systems, such as healthcare, social services, and legal frameworks.
Monitor clients' progress, provide ongoing support, and adjust plans as needed to address changing circumstances.
Facilitate communication and collaboration among clients, family members, caregivers, and service providers.
Maintain accurate and up-to-date documentation of client interactions, progress notes, and service plans.

Qualifications:
Bachelor's or Master's degree in Social Work, Psychology, Rehabilitation Counseling, or a related field (or equivalent experience).
Previous experience in case management, social services, or rehabilitation settings is preferred but not required.
Strong interpersonal skills and the ability to build rapport with clients from diverse backgrounds.
