Position Overview:



  • As a Case Manager (Rehabilitation) Volunteer, you will play a vital role in supporting individuals through their rehabilitation process.

Key Responsibilities:



  • Conduct initial assessments to determine clients' needs, goals, and strengths.

  • Coordinate access to necessary resources, including medical care, therapeutic services, housing, and community support programs.

  • Advocate for clients' rights and needs within various systems, such as healthcare, social services, and legal frameworks.

  • Monitor clients' progress, provide ongoing support, and adjust plans as needed to address changing circumstances.

  • Facilitate communication and collaboration among clients, family members, caregivers, and service providers.

  • Maintain accurate and up-to-date documentation of client interactions, progress notes, and service plans.

Qualifications:



  • Bachelor's or Master's degree in Social Work, Psychology, Rehabilitation Counseling, or a related field (or equivalent experience).

  • Previous experience in case management, social services, or rehabilitation settings is preferred but not required.

  • Strong interpersonal skills and the ability to build rapport with clients from diverse backgrounds.

  • Excellent communication, advocacy, and problem-solving skills.

  • Ability to work collaboratively in a multidisciplinary team environment.

  • Compassion, empathy, and a commitment to promoting clients' well-being and autonomy.

  • Willingness to adhere to ethical standards and maintain confidentiality.
